TDMA Photo Release Wavier
I grant permission to Two Dragons Martial Arts Studio, to take and use: photographs and/or digital images of me for use in news releases and/or educational materials. These materials might include printed or electronic publications, Web sites, or other electronic communications. I further agree that my name and identity may be revealed in descriptive text or commentary in connection with the image(s). I authorize the use of these images without compensation to me.
TDMA Medical Treatment
I hereby give my permission to the staff of Two Dragons Martial Arts, Inc. to have myself, or my child, treated in case of emergency. I fully understand that any medical treatment given to me will be first aid treatment type only. I also understand that if I have any physical problems, I must check with my doctor before taking this program. Two Dragons Martial Arts reserves the right to dismiss any student at any time for misconduct or for actions that would convey an improper image of Two Dragons, its staff, or its students.
